Liability Waiver Agreement: A trade name and operation of Eternity Enterprises, LLC, Soul Liberation Healing & Wellness is not a licensed authority on formal health and does not guarantee any specific results. Classes, events, and workshops conducted by Soul Liberation Healing & Wellness are engaged in at the client's risk and with willful consent. All provided services are voluntary and the client can cease participation at any time. The client is responsible for determining their physical ability to engage in activity. Neither Soul Liberation Healing & Wellness, its owners, officials, workers, teachers, volunteers, and any other associated members, including all property renters and property owners where Soul Liberation Healing & Wellness classes are held, are liable for any injuries or damages resulting from client participation and will not be responsible for any negative outcomes of services, physical or emotional. The client holds full responsibility and agrees to irrevocably release Soul Liberation Healing & Wellness and all aforementioned parties from any and all actions, causes of action, damages, claims, or demands of whatever nature regarding damages, losses, and injuries, known or unknown, which are incurred by, arise from, or are in any way related to participation in a Soul Liberation Healing & Wellness class. The client agrees to not bring forth any legal action against Soul Liberation Healing & Wellness and all aforementioned parties and recognizes that this agreement is also binding on heirs, spouse, children, legal representatives, successors, and assigns. The client agrees to take full responsibility for any minors (defined as children under the age of 18) brought to class and recognizes that all agreement terms apply. The client claims liability as the parent and/or guardian of the minor child/children they bring and claims the full right and authority to execute this release and permission on behalf of the minor(s). Soul Liberation Healing & Wellness reserves the right to refuse service at its discretion. This agreement is binding for present and all future involvements with Soul Liberation Healing & Wellness.
